[Therapy of acute and inveterate ruptures of the tendon of the quadriceps femoris muscle.].

The authors deal with the treatment of acute and inveterate ruptures of the tendon of the quadriceps femoris muscle. They emphasize early diagnosis and surgical treatment in acute conditions. The authors use of modification of McLaughlin's technique which involves trepanation of the patella through which a wire loop is drawn as a relief of the bridging of the sutured site of the tendon, then it is led in a proximal direction through the fibrous portion of the tendon of the m. rectus femoris and terminates about 10 cm in a proximal direction from the site of the rupture. The operation is supplemented by plaster fixation in slight semiflexion of the knee joint - cca 15 degrees - for a period of six weeks. In this stage the patient is engaged only in isometric exercise. When the plaster bandage is removed the patient is hospitalized and starts immediately active flexion and extension of the knee joint and the load is gradually increased in the course of 3-4 days to the normal level. The authors treated thus six patients and achieved very good results - all patients recovered completely active extension of the knee joint to a full extent and minimal flexion of the knee joints of 95 degrees was achieved. In ruptures inveterate the authors use Codivilla's procedure and for partial ruptures they recommend a conservative procedure.
