[Comparison of thromboembolic prophylaxis with heparin and fraxiparine after total endoprostheses of the hip joint.].

The authors compare thromboembolic prophylaxis with Fraxiparine and heparin in patients following implantation of a total endoprosthesis of the hip joint. During the period of investigation extending over 12 month a total of 144 patients were operated; 65 (24 men, 41 women, mean age 67.3 years) were given Fraxiparine and 79 (28 men and 51 women, mean age 68.4 years) were given heparin combined with DH ergotoxin, Curantyl and aspirin. Fraxiparine was administered according to the pattern recommended by the manufacturer. Starting on the fourth day after operation the Fraxiparine dose was increased with regard to the patient's body weight. Fraxiparine was administered alone, heparin retard was administered twice a day combined with DH ergotoxin, Curantyl and aspirin. During the investigation period there was not a single case of deep venous thrombosis or embolism into the pulmonary artery. A haematoma in the wound was observed in one patient who had Fraxiparine and five times among the patients in the heparin group. The heparin pattern was used at the Orthopaedic Clinic in Hradec Králové since 1987. Every year there were several cases of thrombophlebitis and embolism into the pulmonary artery with a fatal outcome. Fraxiparine administered in amounts recommended by the manufacturer reduced in a significant way thromboembolic complications in patients after implantations of a total endoprostheses of the hip joint.
