[The development of early diagnosis and treatment of congenital hip dislocation in Czechoslovakia].

The author gives an account of the early diagnosis and treatment of congenital dislocation of the hip joint in Czechoslovakia since 1918. He pays more detailed attention to the period before the Second World War and the period immediately after the war when aids now used on a world-wide scale were introduced into practice such as Hanausek's apparatus, Frejka's cushion and Pavlik's stirrups. [Experience with 3-stage screening for detection of congenital hip dysplasia].

We report an experience with a three-stage screening programme which has recently been recommended. It comprises the examination of children at three successive periods - at birth, at six weeks and at three months. In a prospective study we evaluated 789 children at all three periods. Pathological (unstable) hips were found at birth in about 20 per cent, the majority being lax hips and all underwent immediate treatment. At six weeks we were unable to detect any sign attributable to hip dysplasia except one instability persisting from birth. The age of the patients was 18 to 40 years (mean age 32 years), the interval after operation was 5 months to 3 years (mean 17 months). In all 8 patients proper covering of the head was achieved, CE angle 25-45 degrees, mean gain of CE angle 35 degrees. The X-ray picture revealed widening of the articular space and reabsorption of arthritic pseudocysts. As regards the clinical picture, marked regression of complaints was recorded. [Use of bone plates in external osteosynthesis].

The author applied the bone plate for external osteosynthesis in 6 patients. Three of them had open fractures of 2nd grade in the area of shin, one patient in the area of forearm. In 2 patients there occurred unstable pseudoarthrosis of shin with significantly trophically changed soft tissue covering. [Pediatric injuries in the lower extremities caused by insertion of the foot into the spokes of a bicycle].

The authors describe the injury of the lower extremity caused by putting the child's leg between the spokes of bicycle wheel during ride. The result is contusion of soft tissues of the leg and ankle or the fracture of the shin in a typical place. The injury requires a through treatment of soft tissues and sometimes also reduction of the fracture under total anaesthesia with subsequent fixation by plaster of Paris. The authors discuss surgical procedures (resections and amputations) in a group of 35 patients with giant-cell bone tumours who were treated at the Orthopaedic Clinic in Hradec Králové. The diagnosis was established in cooperation by the orthopaedist, pathologist and roentgenologist. Resections were made in 24 patients, i.e. in 13 patients as primary operations, in 11 patients as secondary operations. The group comprised three tumours classified as malignant.
