[Use of external ilizarev fixation in severe injuries of the leg.].

T Trc, J Dokoupil

Acta Chir Orthop Traumatol Cech. 1997; 64(2):69-76

The authors summarize their experience with the treatment of some types of complicated fractures of the leg by means of an Ilizarev apparatus. At the authors' department in surgery of fractures of the leg usually an external fixation device is used or intramedullary, locking nailing. In some instances the use of an Ilizarev apparatus proved more useful. This applies to fractures where the line of fractures is in the metaphysis, near the articular space or in case of intraarticular fractures. Ilizarev' s apparatus was used also in patients who had received primary treatment in another department, and who needed for different reasons surgical revision...

[Fractures of the diaphysis of the humerus in children.].

T Pesl, P Havránek

Acta Chir Orthop Traumatol Cech. 1997; 64(2):77-82

Diaphyseal fractures of the humerus are not common in children. With the exception of fractures which occur during delivery they do not differ substantially from diaphyseal fractures in adults as far as the etiology and line of fracture is concerned. They differ, however, substantially as regards treatment. At the Department for Paediatric Surgery, Thomayer Hospital during the period from January 1986 to December 1995, i.e. during a 10-year period, 66 were treated with injuries of the diaphysis of the humerus, which accounts for 6 % of all fractures of the humerus treated at the Department during the investigation period. One fifth was formed by pathological...

[Lesions of lateral ligaments of ankle joint in sports.].

W Orljanski, R Gatterer, G E Wozasek, R Schabus

Acta Chir Orthop Traumatol Cech. 1997; 64(2):83-85

Lesions of the lateral ankle ligaments are most common injuries in sportsmen. Expecially games of ball and racket are of highest risk. Diagnosis consists of a comprehensive clinical examination including tests of stability as well as radiographs to exclude a fracture. Key words: lateral ankle ligaments, functional treatment, ortheses, operative treatment.

[A Combination of Young's and Grice's Operation in Pes Planovalgus in Children with Cerebral Palsy.].

V Smetana, A Schejbalová

Acta Chir Orthop Traumatol Cech. 1997; 64(2):86-89

The authors describe a combination of operations according to Young and Grice, i.e. a combination of suspension of the os naviculare on the anterior tibial muscle for modelling the longitudinal arch of the foot with extraarticular talocalcanear arthrodesis, when dealing with neurogenic deformities of the sole in cerebral palsy in children, and evaluate the results of the operation. In general the authors draw attention to certain changes in the clinical picture of cerebral palsy, in particular a more frequent incidence of deformities of the soles which in recent years shifted from the original age of 6-8 years to the age 4-3 years. The combination...

A new shape memory clamp in small bone surgery.

J Musialek, P Filip

Acta Chir Orthop Traumatol Cech. 1997; 64(2):90-94

A new internal fixative made form a titanium-nickel (TiNi) alloy having shape memory properties has been investigated and manufactured in cooperation between the Municipal Hospital Ostrava-Fifejdy, Department of Orthopaedic Surgery and the TU of Ostrava, Institute of Materials Engineering. The shape memory TiNi clamps with optimized parameters were designed after systematic study and development of required substructure. Experiments with animals supported the philosophy of the TiNi memory clamps application in small bone surgery. The conclusion drawn from preclinical, clinical, radiological and histological evidence from a total of 64 patients is that...

[Experience with the use of hyaluronic Acid in the treatment of arthritis.].

T Trc, M Handl

Acta Chir Orthop Traumatol Cech. 1997; 64(2):95-98

The authors describe their experience with hyaluronic acid (Hyalgan) in the treatment of arthritis in clinical and ambulatory practice. A total of 278 doses of Hyalgan were administered to 169 patients for treatment of arthritis of the knee, ankle, shoulder and hip joint. A certain limitation of treatment was the price of the preparation which had to be paid by the patients. The authors recorded an early subjective effect of the preparation, i.e. regression of pain, improved mobility and improvement of the clinical manifestations of osteoarthritis. It did not prove possible to detect an effect modifying the arthritic manifestations on the joint, although...

[Initial experience with implantation of a cemented endoprosthesis.].

J Cyprich, J Fousek

Acta Chir Orthop Traumatol Cech. 1997; 64(2):99-103

The authors present their experience with the use of a cemented endoprosthesis of the hip joint Saphir I. During a three-year period they implanted at the Orthopaedic department of the Central Military Hospital in Prague 82 of these endoprostheses with very good mediumterm results. In the first part of the paper they deal with the description of the implant and the range of sizes. They emphasize the satisfactory technical solution of the implant and instruments and the easy technique of implantation. In view of the minimal number of complications and the reasonable price they envisage the use this endoprosthesis in elderly but perspective patients...

[Conversion of external fixation to an intramedullary nail in complicated fractures.].

P Závitkovský, J Pilnácek, I Cechová, T Malkus

Acta Chir Orthop Traumatol Cech. 1997; 64(2):104-106

The authors present the method of conversion of external fixation to an intramedullary nail in treatment of complicated fractures of the diaphysis of the leg. This procedure is useful in particular in open fractures and in fractures associated traumatological issue. Conversion gives very satisfactory therapeutic result with a minimum of complications. The purpose of the present paper is to elucidate the procedure and make the readers familiar with the autors' experience assembled with the method in the first six patients. Key words: external fixation, conversion, intramedullary nail.

[Subsequent fate of hip joints after triple screening following neonatal screening.].

J Skrabal

Acta Chir Orthop Traumatol Cech. 1997; 64(2):107-112

The author describes the long-term results of screening of neonatal hip joints on account of congenital dysplasia, using the method of three screening in 1987 and 1988. He examined at the beginning of school attendance clinically and by measuring the angles of the hip joints 94 children with an original post-partum finding or risk children delivered by breech presentation or with a history of inborn dysplasia, and compared the results with those of a control group of 50 healthy hip joints. He found that children with a post-partum finding and those born by breech presentation differed as to the angles of the hip joint significantly from children with...

[Arthroscopy after repeated spraining of the ankle.].

B Delej, S Petrovic, M Papso, P Precinský

Acta Chir Orthop Traumatol Cech. 1997; 64(2):113-116

The authors analyze a group of 43 patients operated at the casualty department of the Ministry of Defence Hospital in Bratislava during the past two years where arthroscopy of the ankle was made. They identify a group of patients who were indicated for arthroscopic surgery after repeated distorsions of the ankle. They compare in these patients the preoperative finding on the native X-ray with the CT finding and finding on operation. Based on postoperative results they try to find an answer to the problem whether arthroscopy of the ankle is indicated after repeated distorsions. Key words: arthroscopy of the ankle joint, distorsion of the ankle joint,...
