[Spotorno CLS Cup in Revision Arthroplasty of the Hip.].

M Krbec, O Cech, V Vrecion

Acta Chir Orthop Traumatol Cech. 2000; 67(6):365-371

UNLABELLED: PURPOSE OF THE STUDY Revision surgery for the loosening of total hip arthroplasty with the use of cemented implants does not guarantee good long-term results. The current trend prefers the use of cementless implants, particularly of the press-fit type. In our group of patients with aseptic loosening of the acetabular cup with a preserved acetabular bone stock without segmental defects we used for the revision surgery a Spotorno CLS cementless cup. MATERIAL The group comprised 33 patients, 29 women and 4 men, of the average age of 66 years, with the loosened acetabular component, of which 31 had originally a cemented Poldi cup and 2 a cementless...

[S-ROM Modular THR System -Three and Half Years of Experience.].

J Bartonícek, L Lunácek, V Fric

Acta Chir Orthop Traumatol Cech. 2000; 67(6):372-381

UNLABELLED: PURPOSE OF THE STUDY Presentation of the first 3,5-year experience with the application of S-ROM system in total hip arthroplasty. MATERIAL A retrospective study evaluating 33 patients (14 men, 19 women), average age 57 years (25-84 years) operated on in the period betwen December 1996 and June 2000. In 5 cases we performed primary implantation and in 28 cases revision surgery. Indications to primary surgery included once primary osteoarthritis, twice postdysplastic osteoarthritis and twice trauma. Of revision surgeries, there were 19 cases of aseptic loosening, 8 cases of septic loosening and once malposition of the stem. In 10 cases the...

Lateral acromion osteotomy: biomechanical comparison of three osteosyntheses methods.

F Kandziora, F Kerschbaumer

Acta Chir Orthop Traumatol Cech. 2000; 67(6):382-386

UNLABELLED: PURPOSE The superior approach to the shoulder joint according to Codman using "sabre cut" and lateral acromion osteotomy, is most convenient for the exposure of the glenoid for arthroplasty. The drawback of this procedure are frequent non-unions of the acromion due to the poor stability of the osteosynthesis. MATERIAL AND METHODS Three osteosynthese methods were tested at random in 18 cadaver scapulae using a Zwick-Universal material testing device (1). Osteosynthesis with 4 screws and a plate according to Grammont (2). Osteosynthesis using special designed Y-plate according to Rozing (3). Osteosynthesis with a tension-wire-band construct...

[Subtalar arthrodesis-surgical management of the planovalgosity of the foot in infantile cerebral palsy.].

M Repko, R Chaloupka

Acta Chir Orthop Traumatol Cech. 2000; 67(6):387-393

UNLABELLED: PURPOSE OF THE STUDY The study evaluated the possibilities of the application of the surgical method of fusion and the mutual comparison of its individual types in the treatment of planovalgus deformity in the foot in spastic patients affected by infantile cerebral palsy. Escalated spasm of eversion muscles of the foot with a simultaneous weakening of tibial muscles resulted in their case in relatively very severe deformities of feet and their poor function. The decision on the type of the chosen arthrodesis (extraarticular talocalcanear or triple subtalar) is based on the bone maturity of the patient and the degree of the possible correction...

[Extended Bauer Approach and Transfemoral Approach in THA Revision.].

O Cech

Acta Chir Orthop Traumatol Cech. 2000; 67(6):394-399

The number of revision surgeries of the hip joint keeps growing and it brings new problems, especially in cases where the skeleton in the region of the loosened stem is destructed. As we have been pursuing total hip arthroplasty in the Czech Republic for 30 years we know that the life of the implants is reduced, the number of aseptic loosening is increasing. This loosening is associated with the loss of the bone stock, mainly the cortex gets thinner. The revision cemented stem cannot be exactly fixed in the destructed proximal femur and, in addition, the bone is further damaged. Wagner introduces a new concept of the treatment of these severe complications:...

[Results of application of hydroxyapatite and hydroxyapatitecollagen.].

F Makai, L Rehák, M Vanis, D Bakos

Acta Chir Orthop Traumatol Cech. 2000; 67(6):400-403

The authors present their experience of the application of hydroxyapatite and a composite-hydroxyapatitecollagen in 2 groups of patients. In the first group they evaluate the mediumterm results in 10 girls and 8 boys of average age of 12,5 years after the filling of bone defects in case of bone cysts, aneurysmatic cysts, fibrous cortical defect and eosinophilous granuloma. Hydroxyapatite was applied in the form of rolls, small bricks, strips. The first signs of healing were evident as early as after 4 months, after 36 months osteointegration was practically finished. In the second group of 1995-2000 period they applied hydroxyapatite in 15 patients...

[Valgus osteotomy of the first metatarsal by a bone graft.].

K Simecek, V Svoboda, V Cecelovský

Acta Chir Orthop Traumatol Cech. 2000; 67(6):404-409

The authors describe their experience of valgus osteotomy of the first metatarsal in hallux valgus - metatarsus primus varus (HV-MTPV). It is based on the Stamm technique consisting in the resection of one third of the base of the proximal phalanx of the great toe. From the resected part they make a wedge-shaped bone graft. Subsequently, they extend the section and perform a two-third transversal osteotomy of the first metatarsal (MT) 1 cm distal to the cuneometatarsal joint. The rest of the bone is broken into valgus position by pressure. Afterwards, the bone graft is inserted in the open osteotomy line from the medial side. Valgus position is fixed...

[Autotransfusion in patients with total hip or knee endoprosthesis.].

L Urbánek, M Pivonková

Acta Chir Orthop Traumatol Cech. 2000; 67(6):410-414

Authors in their paper explain the term autotransfusion, they compare advantages and disadvantages of autologous and homologous transfusion. Perioperative and postoperative blood loss was compared and number of transfused units was recorded in group of 573 patients undergoing total hip or knee replacement. 34.2% of patients had autotransfusion. Authors describe preparation of autotransfusion and its application. They evaluate possible complications of homologous and autologous transfusions and also discuss economical aspects and summarize indications and contraindications of autotransfusion. Finally, they try to make recommendations in order to select...

[Surgical treatment of the ruptured achilles tendon.].

R Hart, M Janecek, P Bucek, P Zelnícek

Acta Chir Orthop Traumatol Cech. 2000; 67(6):415-418

The article deals with the issues of the rupture of Achilles tendon with a focus on its surgical treatment. On the group of 42 patients operated on between 1994 and 1999 with the average follow-up of 3 years and 9 months the authors analyze the epidemiology of this injury, i. e. its incidence by sex, age, causes etc. They also examine the influence of the presence of pretrauma complaints and the associated diseases. The results reflect both the subjective condition and the objective finding. Satisfaction of patients is expressed by a scale of seven subgroups. Achieved were 64,2 points out of the maximum number of 85. Clinical examination is focused...
