[Treatment of thoracolumbar spinal fractures using internal fixators (evaluation of 120 cases)].

M Krbec, J Stulík

Acta Chir Orthop Traumatol Cech. 2001; 68(2):77-84

PURPOSE OF THE STUDY: A group of 120 patients was evaluated with acute injury of Th-L spine who were operated on from posterior approach with the use of Dick AO fixator or USS fixator in the period of 1991-1996.MATERIAL: In the period of 1991-1996, 128 patients were operated on with the acute injury of Th-L spine. Evaluated were 120 patients: 81 men (67.5%) of average age 40.1 years (age range, 16-74 years) and 39 women (32.5%) of average age 35.4 years (age range, 15-66 years). Posterior stabilization using the internal fixator was combined with posterolateral or posterior interarticular fusion. In comminuted fractures of the vertebral body transpedicular...

Z Rozkydal, P Janícek, J Deduch, F Hudecek

Acta Chir Orthop Traumatol Cech. 2001; 68(2):85-92

PURPOSE OF THE STUDY: The aim of the study is to present complications of acetabular component of CLS total hip replacement and their solution.MATERIAL: In the period of 1986-1999 in total 219 cementless CLS total hip replacements and 2012 CLS cups in hybrid replacements were implanted at 1st Orthopaedic Department in Brno--primary surgeries--2026, revision surgeries--205. Of 2231 CLS cups we encountered 25 complications requiring a revision surgery (1.1% of all cases): 2 cases of the broken metal shell, 3 cases of cup penetration into pelvis, 4 cases of cup migration, 5 cases of aseptic loosening, 5 cases of dislocations and 6 cases of deep infections.METHOD:...

Triple osteotomy of the pelvis in children and adolescents.

J Poul, M Vejrostová

Acta Chir Orthop Traumatol Cech. 2001; 68(2):93-98

PURPOSE OF THE STUDY: To assess the results of Steel triple osteotomy on Developmental hip displacement (DDH) and Legg-Calvé-Perthes Disease (LCPD) cases utilising objective criteria based on measurements. All measurements were done by a co-author in a blind way. What concerns DDH, triple osteotomy was used in all cases in which authors felt to obtain a profit for a patient, so that hips with non-spherical head were operated on, too. On the other hand, as far as the experience with Steel osteotomy in LCPD is very limited, authors used this method presuming to obtain similar effect like with Salter osteotomy in younger patients.MATERIAL: 41 Steel...

[Personal experience with treatment of posttraumatic urethral distraction defects].

R Fiala, F Zátura, R Vrtal

Acta Chir Orthop Traumatol Cech. 2001; 68(2):99-104

PURPOSE OF THE STUDY: Authors present their experience in the treatment of posttraumatic distraction urethral defect resulting from traumatic rupture of posterior urethra.MATERIAL: The group comprised 19 patients with posttraumatic urethral distraction defect (average age 41 year, range 27-65 years). In 16 of them (84%) resection urehtroplasty was performed and in three (16%) endoscopic internal urethrotomy was applied. The patients were evaluated of 19 to 48 months after surgery.METHOD: Urethroplasty was performed at least three months after the trauma, always under general anesthaesia in lithotomic position, using perinal approach. Dissection...

Heterotopic bone formation in total hip arthroplasty: predisposing factors, classification and the significance for clinical outcome.

K Schara, S Herman

Acta Chir Orthop Traumatol Cech. 2001; 68(2):105-108

PURPOSE OF THE STUDY: Heterotopic bone formation in the area around the femoral head is a recognised complication following total hip arthroplasty. The aim of the study was to determine the prevalence of heterotopic ossification and to select a method of classification. Also, we studied the potential association of heterotopic bone with the following variables: sex, age at operation, operative technique, type of prosthesis and previous surgery on the ipsilateral hip.MATERIAL: The study involved 103 consecutive patients who underwent total hip replacement at our department in 1995. 175 patients were excluded from the study. Among 103 patients there...

Treatment of defect pseudoarthroses with bone segment transport.

D Delimar, H Klobucar, M Jelić, N Cicak, K Korzinek

Acta Chir Orthop Traumatol Cech. 2001; 68(2):109-111

PURPOSE OF THE STUDY: The most severe injuries during defense war in Croatia were caused by projectiles of high kinetic energy. Shell fragments bone they break the bone, transferring bone segments into additional individual projectiles. During this process periostal and endomedullary circulation is cut off reducing to a minimum the possibility of healing, leading to defects in long bone diaphyses. These are referred to as defect pseudoarthroses and are among the most difficult problems in reconstructive surgery of the musculo-articular system. This article presents bone segment transport as a method of choice for treating defect pseudoarthrosis.MATERIAL:...

[The current approach to liver injuries].

J Zelenák, M Hutan, K Kálig

Acta Chir Orthop Traumatol Cech. 2001; 68(2):112-116

PURPOSE OF THE STUDY: The authors analyze their own three-year material within the treatment (both surgical and conservative) of the injury of liver.MATERIAL: During the given period 18 patients with the injury of liver were treated, of which 15 (83%) surgically and 3 (17%) conservatively. The group included 13 men (72%) and 5 women (28%), the average age at the time of injury was 30 years (range, 19-44 years). Ten patients (56%) suffered a blunt injury and 8 patients (44%) a penetrating injury, of which in 5 patients it was the case of a stab wound and in 3 patients a gunshot wound.METHODS: Indicated to operation were patients with a continuous...
