Surgical treatment and prognostic factors in giant-cell tumor of bone.

M Szendröi, J Kiss, I Antal

Acta Chir Orthop Traumatol Cech. 2003; 70(3):142-150

The rate of local recurrence in the giant-cell tumor of bone (GCT) is influenced by many factors. GCT aggressiveness is related to mitotic activity, aneuploidy of tumor cells, chromosomal abnormality, excessive metalloproteinase expression and alterations in different oncogenes. Statistically, there was no correlation between the recurrence rate and the size and site of tumor, stage of disease and involvement of subchondral bone. However, statistical analysis revealed that the use of adjuvants (phenol, bone cement, etc) and the type of surgical intervention (en block resection versus curettage and grafting) were factors with a prognostic value. Although...

[Surgical treatment of dislocated transverse fractures of the sacrum].

S Taller, R Lukás, P Suchomel, M Krivohlávek

Acta Chir Orthop Traumatol Cech. 2003; 70(3):151-157

PURPOSE OF THE STUDY: Transverse fractures of sacrum are quite rare. Only about 70 cases have been reported in the relevant literature. Low frequency and rather difficult fracture imaging lead to therapeutical uncertainty and are a cause of a delay of urgent surgery.MATERIAL AND METHODS: A group of three male patients aged 17.43 and 31 years was studied. Two patients sustained the injury at a paragliding accident, one by fall from height. One patient underwent only a simple spinal canal decompression 24 days after the injury. The second patient underwent surgery on the fourth day and in the third patient the procedure was carried out sixteen hours...

[Total knee replacement following high tibial osteotomy].

Z Rozkydal, T Pink

Acta Chir Orthop Traumatol Cech. 2003; 70(3):158-163

PURPOSE OF THE STUDY: The objective of the study was to find out to what extent anatomical changes due to high osteotomy of the tibia affect knee function after total knee arthroplasty.MATERIAL: Of the patients with knee osteoarthritis treated by cemented total PFC prostheses in 1995, two groups were selected. Group I consisted of 50 patients who had knee replacement without previous tibial osteotomy; the average age of the patients was 71.2 years and the average follow-up was 5.7 years. Group II comprised 50 patients with high tibial osteotomy before total knee replacement. The average age was 73.5 years and average follow-up was 5.4 years. Before...

[Treatment of anterior glenohumeral instability: personal experience with an arthroscopic stabilization technique, its indications and results].

J Chroustovský, P Malusek, M Jircík, R Konecný

Acta Chir Orthop Traumatol Cech. 2003; 70(3):164-169

PURPOSE OF THE STUDY: Arthroscopic treatment of anterior post-traumatic instability of the glenohumeral joint is a recent surgical procedure. The aim of this study was to evaluate the outcomes of the method and define criteria on which indications for this treatment are based.MATERIAL: Fifty-six active, young patients less than 30 years of age were distributed into three groups according to the method of treatment. Twenty patients were treated conservatively (group A), 12 patients had open surgery (B) and 23 patients underwent arthroscopic stabilization of the glenohumeral joint (C). The average follow-up periods in groups A, B and C were 26, 38...

[Conservative treatment of femoral fractures in children in data from the Orthopedic Clinic of the 2nd Medical Faculty of Charles University].

D Rybka, T Trc, V Mrzena

Acta Chir Orthop Traumatol Cech. 2003; 70(3):170-176

PURPOSE OF THE STUDY: Conservative therapy is the method of choice for treating femoral fractures in younger children all over the world. At the Department of Orthopedic Surgery of the Second Faculty of Medicine, Charles University in Prague, this approach has had a long tradition and has only partly been replaced by other, more recent methods. It is not always necessary to abandon well-established and reliable techniques because of new achievements and this is demonstrated by the results of our study.MATERIAL: In a period of 22 years, 112 boys and 97 girls were treated by Bryant's traction and 62 boys and 46 girls by Weber's traction. The former...

[Use of a silicone metatarsophalangeal joint endoprosthesis in hallux rigidus over a 15-year period].

M Smetana, S Vencálková

Acta Chir Orthop Traumatol Cech. 2003; 70(3):177-181

PURPOSE OF THE STUDY: The aim of this retrospective study was to evaluate the results of a Swanson type, two stem, flexible silicone prosthesis used for first metatarsophalangeal joint replacement in hallux rigidus.MATERIAL AND METHODS: Between June 1987 and December 2001, we implanted this prosthesis in 145 first toes of 128 patients. This group comprised 83 women (96 replacements) and 45 men (49 replacements); the average age of the group was 49.5 (range, 17 to 82) years. Check-ups carried out in 2002 included clinical and X-ray examination and a questionnaire. Eighty-nine patients presented themselves (63 women and 26 men) and eight patients...

[Periosteal tethering of growth plates in long bones (focal fibrocartilaginous dysplasia)].

J Poul, M Straka

Acta Chir Orthop Traumatol Cech. 2003; 70(3):182-186

PURPOSE OF THE STUDY: Another three cases of periosteal tethers (focal fibrocartilaginous dysplasia) are reported. Until now only 54 children affected by this entity were described. Analysis of these three cases concerning clinical appearance, x-ray deformity, preoperative and histologic findings and the end-results of the own method of surgical treatment was undertaken to categorise the proper diagnosis.MATERIAL: The studied group involved distal posteromedial femoral lesion (Boy aged 11 months, Epiphysis-diaphysis angle 68 degrees, femoro-tibial angle 27 degrees). Another case with proximal tibial lesion on the lateral side (Boy aged 18 months,...

[Vertebral osteoid osteoma--peroperative detection of its nidus using a surgical gamma probe].

J Matějka, J Záhlava

Acta Chir Orthop Traumatol Cech. 2003; 70(3):187-190

The case of a vertebral osteoid osteoma localized in the arch and pedicle of the fourth lumbar vertebra in a young girl is described. The condition was manifested by the development of scoliosis, which was also the initial diagnosis. Because of painful muscle contractions and persistent pain at night, osteoid osteoma was suspected and then confirmed by scintigraphy. In order to minimize resection of the posterior stabilizing structures and to ensure removal of the complete nidus, surgery was carried out with the use of a surgical gamma probe. The method is based on administration of a radionuclide (99m Techneciumlabeled methylenediphosphonate) 2 to...
