Acta Chir Orthop Traumatol Cech. 2004; 71(1):6-12
PURPOSE OF THE STUDY: Transarticular C1-2 fixation is a surgical alternative in treatment of atlantoaxial instability. Although the method provides very good immediate and long-term stability, it still involves several disadvantages. The group of patients as reported from various institutions are usually very small and hardly comparable. In order to objectively compare the results of the method, we collected the groups of patients treated in four institutions dealing with surgery of the cervical spine in Czech Republic.MATERIAL AND METHODS: During the 9-years period (1993-2001), the transarticular C1/2 fixation was performed in 80 patients (mean...
[Personal experience with treatment of acetabular fractures].
T Pavelka, J Kortus, M Linhart, J Matĕjka
Acta Chir Orthop Traumatol Cech. 2004; 71(1):13-19
PURPOSE OF THE STUDY: The article presents a retrospective evaluation of a group of 119 patients treated for acetabular fractures between 1996 and 2002.MATERIAL: In the monitored period 119 patients, (89 men, 30 women) average age 35 years, were hospitalized with an acetabular fracture. Surgically treated were 91 patients (70 men, 21 women), average age 29 years. Prevailing in this group were Type A fractures which accounted for 45%, Type B was represented by 35% and Type C included 19%. Twenty-eight patients were treated conservatively.METHOD: Indication for surgical treatment was instability, hip incongruence or combination of both. Conservative...
[Is osteolysis associated with a stable total hip replacement asymptomatic?].
J Gallo, P Kamínek, J Zapletalová, I Cechová, J Spicka, R Ditmar
Acta Chir Orthop Traumatol Cech. 2004; 71(1):20-25
PURPOSE OF THE STUDY: It has been reported that periprosthetic osteolysis in a stable cementless total hip prosthesis is often free from symptoms. The aim of this study is to check this assumption and provide details on the clinical presentation of this troublesome and worrying complication of hip arthroplasty.MATERIAL: Between March 1999 and June 2002, 76 hips in 72 patients underwent revision arthroplasty for periprosthetic osteolysis associated with stable ABG I prostheses. This patient group included 53 females and 19 males. The average age at the time of revision was 49 years (range, 32 to 63). The average time between the primary and revision...
[Ten years' experience with our method of wrist joint arthrodesis].
J Pech, A Sosna, S Popelka, D Veigl
Acta Chir Orthop Traumatol Cech. 2004; 71(1):26-30
PURPOSE OF THE STUDY: Total arthrodesis of the carpal joint is a complex operative procedure that results in the restoration of joint stability in frontal and sagittal planes and improvement in function of the tendons of digits' extensors and flexors. We developed our own method based on the use of a special implant; in this study we report our long-term results.MATERIAL: Since 1992 we have carried out 54 total carpal arthrodeses, using our method, on 51 patients; three patients were treated bilaterally. Both wrists were treated in. This group comprised 34 women and 19 men, with the average age of 47.6 years. We used the modified method mainly...
[Evaluation of results after reimplantation of total hip arthroplasty cemented cup].
V Dzupa, O Cech, V Pacovský, P Pazdírek
Acta Chir Orthop Traumatol Cech. 2004; 71(1):31-36
PURPOSE OF THE STUDY: The aim of the study is to evaluate the results achieved after the replacement of a loosened or broken cemented cup by another cemented cup and to specify conditions under which such procedure may be successful.MATERIAL: In the period of 1992-1997, 158 revision surgeries of total hip arthroplasty were performed at the authors' Department. In 69 patients (57 women, 12 men) the cemented cup was replaced by another cemented cup and the original femoral monocomponent with the 32 mm head diameter the surface of which was not damaged was left in place.METHODS: The indication for operation was loosening of Degree 2 or 3 of the...
[Post-traumatic anterior shoulder instability--arthroscopic stabilization method using bone anchors].
M Reichl, K Koudela
Acta Chir Orthop Traumatol Cech. 2004; 71(1):37-44
PURPOSE OF THE STUDY: To evaluate the short-term results of re-insertion of the ventral labrum associated with plication of the capsular structures in anterior post-traumatic instability of the shoulder.MATERIAL: The patients treated in our institution between April 2000 and December 2002 and followed up for periods ranging from 3 to 20 months were included. A total of 65 patients were treated for post-traumatic ventral instability. Open capsular shift, after a final evaluation of preoperative examination under total anesthesia and arthroscopic inspection of the labroligamentous complex affected, was used in four patients. Arthroscopic stabilization...
[Arthroscopic subacromial decompression--personal experience and results].
L Urbánek, V Karjagin
Acta Chir Orthop Traumatol Cech. 2004; 71(1):45-49
PURPOSE OF THE STUDY: The aim of the study was to evaluate the outcomes of a comparatively new surgical technique--the arthroscopic subacromial decompression, justify its use and give reasons for refraining from open acromioplasty after Neer. The evaluated clinical sample comprised a relatively large group of patients, the biggest one presented so far in the Czech literature. The authors point out the benefits of this procedure and on the basis of the outcomes recommend its further use.MATERIAL: In the period of January 1999 through November 2001 we performed arthroscopic subacromial decompression for primary or post-injury impingement syndrome...
[Isolated fractures of the atlas].

Acta Chir Orthop Traumatol Cech. 2004; 71(1):50-55
PURPOSE OF THE STUDY: To present the current tends in the diagnosis and management of isolated atlas fractures based on the retrospectively evaluated group of patients with this trauma.MATERIAL: In the period from 1995 to 2002, we treated 486 injuries to the cervical spine at our department. Out of these, 19 patients sustained an isolated fracture of the first cervical vertebra. This group consisted of 12 men and seven women; the average age was 46.6 years. Neurological findings in 18 patients were classified as Frankel E and, in one, as Frankel A. The causes of injury included a fall from height in five patient, a fall in the street in five pedestrians,...
