A Comparison of Operative Invasiveness in Minimally Invasive Anterolateral Hip Replacement (MIS-AL) and Standard Hip Procedure, Using Biochemical MarkersOriginal papers

D. MUSIL, J. STEHLÍK, M. VERNER

Acta Chir Orthop Traumatol Cech. 2008; 75(1):16-20 | DOI: 10.55095/achot2008/003

PURPOSE OF THE STUDYThe aim of this prospective randomized study was to compare, by means of biochemical markers, the operative invasiveness of the standard total hip replacement with that of the minimally invasive anterolateral (MIS-AL) approach.MATERIALTwenty-six randomly assigned patients with standard and 22 patients with MIS-AL total hip replacement were included in the study. Patients with elevated pre-operative levels of the markers evaluated or patients taking medication that might affect marker levels were not included.METHODSCreatine phosphokinase (CPK) and C-reactive protein (CRP) were chosen as markers of muscle damage...

Initial Experience Using the Inverse Prosthesis of the ShoulderOriginal papers

Acta Chir Orthop Traumatol Cech. 2008; 75(1):21-27 | DOI: 10.55095/achot2008/004

PURPOSE OF THE STUDYThe aim of our retrospective study is to show that in cases of very serious degenerative alteration of the shoulder as well as massive Cuff arthropathy, there is a valid reason for the indication of an inverse prosthesis of the total shoulder joint. We therefore describe operation technique as well as evaluation of our own results, including complications.MATERIALFrom October 2000 to October 2005, 24 patients had 25 Delta prostheses implanted. All these 25 implantations were carried out by the author personally, using the Delta prosthesis, in Hybrid technique.METHODIn all 25 implantations we used a lateral...

Conservative Treatment Using Plasma Rich in Growth Factors (PRGF) for Injury to the Ligamentous Complex of the AnkleOriginal papers

Acta Chir Orthop Traumatol Cech. 2008; 75(1):28-33 | DOI: 10.55095/achot2008/005

PURPOSE OF THE STUDYThe authors describe the therapeutic utilization of separated/isolated autologous growth factors in semiconservative treatment of type III injury to the ankle ligamentous complex.MATERIALBetween October 2004 and March 2005 a group of 11 patients, two women and nine men, aged 18 to 41 (average, 25.09) years with acute injury to the lateral ligamentous complex of the ankle were treated by plasma rich in growth factors (PRGF) infiltration. On functional radiographic examination, the post-traumatic lateral opening of the tibiotalar intraarticular space was 17.45 degrees (range, 12.0-30.0; s = 5.68).METHODSThe injured...

Surgical Management of Posterior Cruciate Ligament Avulsion FractureOriginal papers

Acta Chir Orthop Traumatol Cech. 2008; 75(1):34-39 | DOI: 10.55095/achot2008/006

PURPOSE OF THE STUDYInjury to the posterior cruciate ligament (PCL) is relatively rare and, if combined with avulsion fracture of the PCL insertion site, it accounts for only a small number of knee injuries. This is why PCL avulsion fracture is an easily missed diagnosis resulting in knee instability and arthritis development. The aim of this study was to show the necessity of surgical treatment of these injuries.MATERIALBetween January 2004 and September 2005, four patients with PCL injury underwent surgery. Three had avulsion fracture at the PCL insertion site, and in one the avulsion fracture involved also the intercondylar eminence....

Acta Chir Orthop Traumatol Cech. 2008; 75(1):40-47 | DOI: 10.55095/achot2008/007

PURPOSE OF THE STUDYSubtotal or total meniscectomy will increase weight-bearing per square unit of the cartilage surface approximately three-and-half-times. A long-term overloading of cartilage is clinically manifested by pain, swelling and a rapid onset of early arthritic lesions discernible on radiograms. One of the options for the treatment of degenerative changes in the joint is meniscal transplant. The authors present their first experience with the transplantation of deep frozen meniscal tissue in the Czech Republic.MATERIALBy September 2006, we had treated 26 patients with clinical problems following subtotal or total meniscectomy....

Results of the Treatment of Odontoid Fractures by Osteosynthesis with a Single Axial ScrewOriginal papers

K. SAUR, M. SAMEŠ

Acta Chir Orthop Traumatol Cech. 2008; 75(1):48-51 | DOI: 10.55095/achot2008/008

PURPOSE OF THE STUDYThe authors present the clinical results of odontoid fracture treatment, using a single axial screw, with the aim to compare their results with the findings of other relevant studies, with focus on the use of either one or two axial screws.MATERIALThirty patients were included in the study, 16 men and 14 women at an average age of 56.1 years. They had findings of type II and "shallow" type III fractures, as evaluated on the basis of Anderson and D'Alonzo classification, and combined fractures. All were managed by the method of direct osteosynthesis of the dens with a single axial screw from the anterior approach in the...

Treatment of Distal Humerus FracturesCurrent concepts review

Acta Chir Orthop Traumatol Cech. 2008; 75(1):6-15 | DOI: 10.55095/achot2008/002

The elbow is a complex joint and is vital in positioning the hand in space. We believe that open reduction internal fixation offers the best chance for return to function following intra-articular fractures of the distal humerus. We advocate the following principles for the effective treatment of these injuries: identification and protection of the ulnar nerve followed by transposition, broad exposure of the fracture utilizing an olecranon osteotomy, anatomic restoration of the articular surface with preservation of all osteochondral fragments, rigid fixation of both columns using pre-contoured plates and screws, and the institution of early range...

Acta Chir Orthop Traumatol Cech. 2008; 75(1):52-60 | DOI: 10.55095/achot2008/009

AO Principles are described today as follows:* appropriate reduction* appropriate stabilization* preservation of vascularity* painless early mobilizationThese requirements can all be met by intramedullary osteosynthesis for suitable indications. The modern generation of nails is highly user-friendly and application of the systems is very safe. Outcome predictions favor reamed locked nailing for suitable fractures, whereby reaming should be moderate. The need to activate the dynamization option should be evaluated six to eight weeks postoperatively.
