PURPOSE OF THE STUDYTo present the results of an independent mono-centric prospective study on patients with a mobile ProDisc-C implant. This cervical artificial disc replacement (CADR), which is one of the options for avoiding cervical spine fusion, was evaluated during two-years follow-up.MATERIALA total of 61 patients underwent CADR with a ProDisc-C in one or two segments at the Department of Spinal Surgery of the University Hospital in Motol, Prague, in the period from October 2004 to May 2006. Of these, 39 were included in the study and followed up for 2 years at least. Except for one patient, one segment was replaced in all patients.METHODSThe...

PURPOSE OF THE STUDYTo present a prospective evaluation of one-year results in 162 patients undergoing total hip arthroplasty from a minimally invasive antero-lateral approach (THA MIS-AL).MATERIALA total of 249 THA MIS-AL procedures were performed between January 2005 and October 2007. At one year after surgery 162 patients were examined and their conditions evaluated by the Harris score, percent satisfaction assessment, 1-to-5 scale school marking, findings on lateral X-ray images and occurrence of post-operative complications.METHODSWith the patient lying in a lateral recumbent position, access is gained through a short incision...

PURPOSE OF THE STUDYThe most frequent deformity of the big toe and forefoot associated with a collapse of the transverse arch of the foot is a valgus deformity. For correction of a hallux valgus, several procedures are described in the literature. A valgus deformity often develops due to a varus deviation of the first metatarsal bone when the intermetatarsal angle between the first and second metatarsals is greater than 10 degrees. When the intermetatarsal angle is larger then 10 degrees or the first ray is hypermobile, a Lapidus procedure is one of the options. The objective of this study was to evaluate the outcomes in patients with halux valgus...

PURPOSE OF THE STUDYDevelopmental dysplasia of the hip (DDH) is a serious anatomical abnormality. The aims of the study were to compare the current views on general screening for DDH and, in a group of patients diagnosed for DDH both clinically and ultrasonographically, to analyse the results of their treatment with Wagner stockings.MATERIAL AND METHODSIn the years 2000 to 2001, a total of 3,221 children were examined at birth clinically and by ultrasonography using Graf's method. DDH was diagnosed in 137, and Wagner stockings were used for treatment in 127 patients. This Wagner abduction device consists of linen stockings buttoned to a...

PURPOSE OF THE STUDYTo present the results of total wrist replacement with a cementless prosthesis designed by us.MATERIAL AND METHODSA group of five men and 27 women treated between 2004 and 2007 was evaluated. The average age was 51.6 years and follow-up ranged from 4 to 38 months, with an average of 9.4 months. Indications for surgery included wrist destruction due to rheumatoid arthritis, arthritis or psoriatic arthropathy.Subjective evaluation recorded satisfaction of the patients with the wrist range of motion, cosmetic appearance of the hand and pain relief. The range of motion was assessed with a goniometer at 4 months of follow-up...

PURPOSE OF THE STUDYIn the development of cementless total hip arthroplasty wear, loosening as well as stress shielding are considered as major issues. New results in literature specify survivorship of THA over 97%. Consequently the implant loosening and wear especially can be considered as almost solved. Therefore, it is essential to use bone preserving primary implants that allow for a physiological load transfer and cause no or only slight stress shielding at the proximal femur. The MAYO conservative hip stem with a wedge design ensuring immediate primary fixation of the stem with metaphyseal load transfer.MATERIAL AND METHODSA retrospective...

Our understanding of fracture healing has undergone an evolution over many decades with continuous improvement of fracture treatment. Solid union is a precondition of restoring the function of a fractured bone. The goal of the early treatment of the fracture was focussed upon enabling solid union in acceptable alignment of the fracture. This was achieved with reduction followed by application of external splints. The function of the articulations was often troubled by long lasting and extensive external immobilization, which required physiotherapy that lasted longer than bone union. The surgical reduction and stabilization aimed at early recovery...

The authors analyze one of the new therapeutic approaches - the use of autologous growth factors. Because the chemical structure of these proteins and their involvement in many functions of the organism affect the whole range of tissues, with a possibility to use them in human medicine. Growth factors, such as platelet-derived growth factor, transforming growth factor and others, exert effects on fibroplastic events during ontogenesis as well as on regeneration of tissues injured due to an accident or surgery. The preparation of therapeutic doses of growth factors consists of autologous blood collection, plasma separation and application of plasma...

Based on case histories, the authors draw attention to important infectious complications in patients surgically treated for pelvic injuries. These complications were recorded in four out of 75 patients (5.3 %) undergoing surgery for pelvic fracture in the period from 2001 to 2005. The chief risk factors for the development of infection included the poor state of health of a polytraumatized patient in combination with a long stay in an acute care unit, and severe trauma and bleeding of soft tissue structures in B and C types of pelvic injury. Bacteriological findings showed the presence of several pathogenic species (Enterobacter cloacae, Citrobacter...

The authors analyse the first results of correction surgery for limb-length inequality and rotational and angular deformities of the tibia using the Taylor spatial frame in combination with the Ilizarov external fixator in three patients. The shortening of the tibia, 65 mm on the average (55, 60 and 80 mm, respectively), was due to tibial hemimilia in two patients and traumatic epiphyseolysis of the distal tibia in one patient. Distraction at a rate of 1 mm/day was controlled three-times a day on two contralateral struts. It started on the seventh day after osteotomy in all three patients. On the second post-operative day, rehabilitation started with...

The aim of this case report is to present an unusual double synovial cyst that arose from the proximal tibiofibular joint compressing the peroneal nerve in the peroneal tunnel and was unrecognized at the beginning. According to the review of literature back to 1891, only 62 cases of cysts originating from the proximal tibiofibular joint (PTFJ) have been described. We report a case of a 32 year old male patient who was admitted to the Department of Orthopaedic Surgery because of a classic peroneal tunnel syndrome of the left leg. On the lateral side of the proximal third of his left leg a tumefaction of 12 x 2.5 cm was visible. The sonography showed...

This case report presents the results of a conversion hip arthroplasty after a long lasting Girdlestone situation of 15 yaers in combination with an extensive shortening of the leg of 7.2 centimeters. An extensive subtrochanteric shortening osteotomy of 3.5 centimeters was necessary to restore leg length and the rotational centre of the hip.Functional outcome was excellent within the long run and a restoration of the independence of the patient was achieved.
