Acta Chir Orthop Traumatol Cech. 2008; 75(5):325-331 | DOI: 10.55095/achot2008/060
PURPOSE OF THE STUDYSegmental tibial fractures are commonly believed to be more difficult to manage, requiring surgical treatment. Our experience with forty-eight segmental tibial fractures suggests that these fractures, if closed and with shortening of an acceptable initial degree and a corrected deformity, may be successfully treated with functional braces.MATERIAL AND METHODSForty-eight closed segmental fractures of the tibia that had initial shortening ≥12 mm and angular deformity manually corrected to ≥7 degrees were stabilized in an above-the-knee cast for a median time of 33 days and a mode of 15 days. They were subsequently...

Acta Chir Orthop Traumatol Cech. 2008; 75(5):332-338 | DOI: 10.55095/achot2008/061
PURPOSE OF THE STUDYThe conservative treatment of an odontoid fracture with immobilization in a halo-vest or collar often results in pseudoarthrosis. Therefore, surgical treatment is preferred, and the Magerl-Böhler anterior osteosynthesis of the C2 dens is one of the options for achieving good bony union. The aim of this study was to show that the success of reducing an odontoid fracture is related to the direction of fracture lines and that of displacement, and that anterior osteosynthesis of the C2 dens provides sufficient stability when fixed either with one or two screws.MATERIALPatients treated for odontoid fracture at the Department...

Acta Chir Orthop Traumatol Cech. 2008; 75(5):339-346 | DOI: 10.55095/achot2008/062
PURPOSE OF THE STUDYHydroxypatite coating (HAC) was introduced into total hip arthroplasty (THA) practice to improve the fixation interface between bone and prosthesis. To test this assumption however, long-term follow-up investigations are needed. In this study, we present data for two consecutive series of THA stems with HAC and a minimum ten-year follow-up.MATERIALOverall, 249 patients (271 hips) were included in the study, of these 122 (135 hips) had Walter hip arthroplasty (WHA group) with a two-layered TiO2/HAC at the proximal part of the stem and 127 (136 hips) had ABG I prostheses (ABG I group) with a single-layered HAC at the proximal...

Acta Chir Orthop Traumatol Cech. 2008; 75(5):355-362 | DOI: 10.55095/achot2008/064
PURPOSE OF THE STUDYIsolated or combined surgical procedures on muscles around the hip joint are currently indicated by many authors. In cerebral palsy patients they are regarded as essential intervention.MATERIALIn the years 2005-2007, surgery in the hip joint region was essential for 150 children between 3 and 18 years of age. At the time of surgery, the patients' locomotion ranged from stage 1 to stage 7 of the Vojta system.METHODSThe outcome was evaluated by clinical and radiographic examination at 2 and 6 months post-operatively and hip migration percentage and Wiberg's CE angle were measured.RESULTSThe best clinical...

Acta Chir Orthop Traumatol Cech. 2008; 75(5):363-368 | DOI: 10.55095/achot2008/065
PURPOSE OF THE STUDYArthroscopic treatment of calcaneal spur syndrome is a tissue-sparing and effective approach when conservative therapy has failed. This method, its results and our experience with the treatment of this syndrome are presented here.MATERIALBetween January 2003 and November 2007, 26 patients underwent an arthroscopic procedure for calcaneal spur syndrome; of these, 20 were women with an average age of 49 years, and six were men with an average age of 45 years. Four, three women and one man, were lost to follow-up, therefore 22 patients with 24 heels were eventually evaluated. All had conservative therapy for 3 to 6 monts.METHODSThe...

Acta Chir Orthop Traumatol Cech. 2008; 75(5):387-391 | DOI: 10.55095/achot2008/069
Long-term or recurrent osteomyelitis results in the deterioration of soft tissue quality at the site of an infectious process. An insufficient perfusion of the scar tissue contributes to difficult treatment of osteomyelitis. A local transpositional or free muscle flap used to replace the vulnerable scar tissue will enhance perfusion of the affected bone and will help to control osteomyelitis. The authors present the case of a patient who had been treated by bone transport for a 14-cm-long segmental defect of the tibia after post-traumatic osteomyelitis. Recurrent osteomyelitis was found nine years after the primary surgery. The whole tibial cavity...

Acta Chir Orthop Traumatol Cech. 2008; 75(5):392-395 | DOI: 10.55095/achot2008/070
We describe a case of 30-year old male followed-up since the age of 6 for severe type of rare combination of polyarticular form of pigmented villonodular synovitis with hereditary malformation constellation consistent with Noonan-like syndrome. Within 20 years, the patient underwent repeated synovectomies of large joints with temporary effect only. Radiation therapy with the dose of 30 Gy in 15 sessions has been applied for active aggressive synovitis of both knees associated with pain and progressive joint destruction. Favorable effect lasted for 3 years. Progressive destruction with range-of-motion limitation required successive total joint replacement...
