Acta Chir Orthop Traumatol Cech. 2016; 83(2):74-83 | DOI: 10.55095/achot2016/012

PURPOSE OF THE STUDYTotal joint replacement is one of the options in surgical treatment of advanced ankle arthritis. It allows the ankle to remain mobile but, unfortunately, it does not provide the same longevity as total knee or hip replacements. Therefore, decisions concerning the kind of treatment are very individual and depend on the clinical status and opinion of each patient.MATERIAL AND METHODSA total of 132 total ankle replacements were carried out in the period from 2004 to 2015. The prostheses used included the Ankle Evolutive System (AES) in 52 patients, Mobility Total Ankle System (DePuy) in 24 patients and, recently, Rebalance...

Acta Chir Orthop Traumatol Cech. 2016; 83(2):84-87 | DOI: 10.55095/achot2016/013

PURPOSE OF THE STUDYThe aim of this prospective study; The Trauma Assisted Discharge Scheme (TADS), was to set up a new model of post-operative care for patients following a fractured hip, addressing the need for efficiency, cost effectiveness and meeting local demand.MATERIAL AND METHODSAll patients with hip fractures between December 2010 and December 2011, meeting the TADS inclusion criteria were enrolled in the study. Innovative use of staff within existing budgets helped create a TAD team who with the use of defined patient goals and a link nurse provided a seamless transition from acute to community services.RESULTSOne hundred...

Acta Chir Orthop Traumatol Cech. 2016; 83(2):94-101 | DOI: 10.55095/achot2016/014

PURPOSE OF THE STUDYThe aim of the study was, first, to assess satisfaction of our patients after primary total knee arthroplasty (TKA) at various intervals of follow-up. Second, a list of pre- and peri-operative factors with potential effects on patient satisfaction was analysed. The third objective was to identify conditions under which a result of replacement could be considered definite with a little probability of further unexpected development. A more detailed knowledge of unsatisfied patients may provide valuable information for use in pre-operative evaluation, indication for surgery, as well as in the surgery itself.MATERIAL AND METHODSThis...

Acta Chir Orthop Traumatol Cech. 2016; 83(2):102-105 | DOI: 10.55095/achot2016/015

PURPOSE OF THE STUDYAlthough supracondylar humeral fractures represent a major part of the pediatric fractures, no classification system or radiological characteristics describes which supracondylar fractures require open reduction. We aim to evaluate the factors that lead us to perform open reduction during operation.MATERIAL AND METHODSWe retrospectively evaluated 57 patients who underwent operation for type III supracondylar fracture, and divided them into two groups; those with open reduction and internal fixation, and those with closed reduction and percutaneous fixation. The two groups were compared based on age, gender, BMI by age,...

Acta Chir Orthop Traumatol Cech. 2016; 83(2):106-110 | DOI: 10.55095/achot2016/016

PURPOSE OF THE STUDYThe aim of the study was to evaluate, on the basis of radiographic findings and AOFAS scores, the results of the Grice extra-articular subtalar arthrodesis for treatment of planovalgus foot deformity in cerebral palsy patients.MATERIAL AND METHODSA total of 38 patients (50 feet) with cerebral palsy indicated to the Grice procedure for planovalgus foot deformity between 2006 and 2010 were assessed. The group comprised 18 girls and 20 boys, of whom 10 had spastic quadriparesis (four undergoing bilateral surgery), three had triparesis, four had hemiparesis and 21 had diparesis (treated on both sides in eight). The average...

Acta Chir Orthop Traumatol Cech. 2016; 83(2):111-116 | DOI: 10.55095/achot2016/017

PURPOSE OF THE STUDYIn a long-term perspective, it is better to remove implants after fracture healing. However, subsequent full or excessive loading of an extremity may result in refracture, and the bone with holes after screw removal may present a site with predilection for this. The aim of the study was to find ways of how to decrease risk factors for refracture in such a case. This involved support to the mechanical properties of a bone during its remodelling until defects following implant removal are repaired, using a material tolerated by bone tissue and easy to apply. It also included an assessment of the mechanical properties of a bone...

Effect of Negative Pressure Wound Therapy on Infectious Complications in Grade III Open FracturesOriginal papers

Acta Chir Orthop Traumatol Cech. 2016; 83(2):117-122 | DOI: 10.55095/achot2016/018

PURPOSE OF THE STUDYGrade III open fractures are associated with infectious complications in 25-66% of injuries. Negative pressure wound therapy (NPWT) applied to an injured soft tissue coverage provides an impermeable barrier between the injured structures and the external environment, in addition to early secretion draining and a positive effect on the site of application. All this also prevents secondary bacterial contamination. The objective of the study was to compare the results of treatment methods in view of infectious complications in patients with soft tissue injury in grade-III open fractures managed either by NPWT and primary closure...

Acta Chir Orthop Traumatol Cech. 2016; 83(2):123-126 | DOI: 10.55095/achot2016/019

PURPOSE OF THE STUDYThoracic trauma, one of the most frequent injuries in patients with multiple traumata, is found in 50 to 80% of these patients and it is crucial for the patient's prognosis. It accounts for 25% of all death from polytraumatic injuries. The aim of this retrospective study was an analysis of the occurrence of chest injuries in polytrauma patients and their surgical treatment in the Trauma Centre or Department of Surgery of the University Hospital Pilsen in a five-year period.MATERIAL AND METHODSPatients with injuries meeting the definition of polytrauma and an Injury Severity Score (ISS) ≥16 were included. The demographic...

Pyoderma Gangrenosum Mimicking Necrotising Fasciitis after Rotator Cuff ReconstructionCase report

J. VČELÁK, R. ©UMAN, J. BENE©

Acta Chir Orthop Traumatol Cech. 2016; 83(2):127-130 | DOI: 10.55095/achot2016/020

Gangrenous pyoderma is a rare complication of surgical treatment mimicking bacterial necrotizing fasciitis. The characteristic symptoms include a pre-existing immune disorder, negative results of the relevant microbiological examinations, no response to any antibiotic therapy, and deteriorating of the condition after radical debridement. Another typical feature is a discrepancy between an ominous appearance of the wound accompanied by leucocytosis and an elevated CRP level, and the patient s good general condition with low-grade fever and no signs of sepsis. The case of a 68-year-old man who underwent an elective open reconstruction of rotator cuff...
