PURPOSE OF THE STUDYThe aim of this study was to present the results of treatment of deep chondral defects of the patella by transplantation of a solid chondral graft formed by autologous cultured chondrocytes.MATERIAL AND METHODSIndications for autologous chondrocyte implantation most frequently included acute trauma to the knee, particularly the patella, which resulted in a severe focal cartilage defect. The lesions were assessed on the basis of the Bessette and Hunter classification preoperatively, and on the Outerbridge classification during arthroscopy. A sample of healthy cartilage was harvested from a non-weight-bearing area of the...

PURPOSE OF THE STUDYThe aim of the study was to find out, by magnetic resonance (MR) imaging, the degree of blood supply impairment in the femoral head in proximal femur fractures.MATERIALTwenty-eight patients, 22 women and six men, aged between 49 and 96 years (average, 75.2 years) were evaluated. Twenty-six patients were examined within 2-48 hours of injury, one patient at 72 hours (3 days) and one at 144 hours (6 days). Two groups were made: A, including 18 patients with an intra-articular fracture of the femoral neck, and B, consisting of 10 patients with an extra-articular fracture (per- to subtrochanteric fractures) for comparison.METHODSNative...

PURPOSE OF THE STUDYIn Part 1 of this study we evaluate the results of surgical repair of massive rotator cuff tears by a "mini-open" technique. In Part 2 we will compare them with the results of reconstructions performed by arthroscopic surgery.MATERIALBetween 1995 and 2006, 99 repairs of massive rotator cuff (RC) tears were carried out in our department. The patient group included 73 men and 23 women at an average age of 55.7 years (range, 37 to 74 years). In 69 cases surgery was performed on the dominant (right) upper extremity.Surgical repair was indicated on the basis of clinical, radiological and arthrographic examination of the...

PURPOSE OF THE STUDYThe aim was to evaluate the diagnostic value of plain X-ray images in the diagnosis of pelvic ring injury in comparison with CT findings.MATERIAL AND METHODSThe group evaluated consisted of 30 patients, 11 women and 19 men, at an average age of 46 years (range, 20-81 years; women, 20-68 years, men, 20-81 years; average age, 49 and 44 years in women and men, respectively) in whom plain radiography and CT scans were done as part of the initial examination. A retrospective assessment of plain X-ray images was carried out by two independent specialists (radiologist and trauma surgeon). The authors together evaluated CT scans...

The authors compare their experience with the treatment of pelvic ring injuries with the literature data concerning the etiology, diagnosis and classification of this trauma, co-existing pathologies, primary therapeutic procedures, timing of the definitive treatment, surgical approaches, osteosynthesis of the posterior and anterior segments, complications and lasting sequelae. The authors regard the issue of surgical treatment as an independent discipline in the field of musculoskeletal system traumatology. They see the reason for it in the variability and complexity of primary therapeutic procedures and the necessity of following up and attending...

Three-dimensional (3D) sonography is the next logical step in diagnostic ultrasound examination. The true value of 3D ultrasonography, however, becomes evident only if 3D structures can be assessed without preconceptions ensuing from 2D interpretations. 3D ultrasonography can greatly improve our understanding of locomotor apparatus anatomy and pathology. The authors used spatial analysis to evaluate the data obtained by examination of patients with orthopedic diagnoses. The Voluson 530 MT and SONOReal system were used for examination. The Voluson permits a choice of either a 2D or a 3D imaging program for musculoskeletal system examination. The...

Conservative treatment of pathologic fractures of the long bones have been reported very infrequently, especially when fracture is caused by an tumour. This report highlights the possibility of an nonoperative treatment of a pathologic humerus fracture caused by an cartilaginous tumour with radiographic criterions of an chondrosarcoma.

Odtržení interkondylické eminence tibie - původní artroskopická metoda fixace pomocí K-drátůKazuistika

Acta Chir Orthop Traumatol Cech. 2006; 73(6):403-404 | DOI: 10.55095/achot2006/062

An original arthroscopic method of osteosynthesis for treatment of tibial tubercle avulsion is presented and documented on the histories of four patients. The avulsed portion of the tibial tubercle was fixed with two Kirschner wires bent intraarticularly to create hooks for fragment fixation. The K-wires were drilled with the use of a tibial aimer for anterior cruciate ligaments. A brace was applied for 1 to 6 weeks. The implants were removed at 8 to 13 weeks by arthroscopy. During their extraction carried out under anesthesia, one patient had a positive Lachmann test of 5 mm. All patients returned to preoperative activities. The mode of fixation reported...

Rehabilitation is an important part of therapy in patients who have had arthroscopic anterior cruciate ligament reconstruction. A well-designed rehabilitation program avoids potential graft damage and speeds up patients' return to their full function level. The course of rehabilitation depends on the type of surgery, mode of fixation and possible co-existing injury to the knee's soft tissues. The rehabilitation program presented here is based on the present-day knowledge of neurophysiological and biomechanical principles and is divided into five phases. In the pre-operative phase (I), the main objective is to prepare patients for surgery in terms...
