PURPOSE OF THE STUDYAlthough great advances have been made in both radiological diagnosis and antibiotic therapy of microbial infections, the treatment of spinal infections remains a major clinical challenge. Many of the patients affected are referred to spinal units with long delays. The general population is ageing and the number of immunocompromised patients, as well as the number of operative procedures for spinal disorders are increasing. The aim of our study was to evaluate the clinical presentations of spinal infections, options for their diagnosis, indications for treatment and their risk factors and the results of surgery.MATERIAL...

PURPOSE OF THE STUDYIn this study the results of arthroscopic repair of massive rotator cuff tears are evaluated and compared with those of mini-open surgery published in Part 1.MATERIALBy the year 2006, of 176 patients undergoing the reconstruction of massive rotator cuff tears in our department, 77 were treated by arthroscopy. In this group there were 50 men; the dominant arm was operated on more frequently (60x). The average age of the patients was 55 years (range, 37-74).METHODSSurgery is carried out under combination of general anesthesia and an interscalene brachial plexus block, in a lateral recumbent position, with traction...

PURPOSE OF THE STUDYThe aim of the study was to ascertain what proportion of patients undergoing total knee arthroplasty (TKA) complain of restricted knee joint motion, and to investigate options for improvement of this situation.MATERIALOur evaluation included a group of 796 patients treated with TKA at our department in the period from January 1, 1990, to December 31, 2004. In all cases, a condylar implant with preservation of the posterior cruciate ligaments was used.METHODSIn addition to medical history, the range of motion, knee joint malalignment and radiological findings were assessed before surgery. After THA, the type of...

PURPOSE OF THE STUDYThe utility of scoring systems, which are used to determine health status or treatment benefit in patients with knee osteoarthritis is under discussion. Therefore it was the purpose of our investigation to evaluate the reliability and the concordance of two established knee scoring systems.METHODSThirty-eight patients with unilateral knee osteoarthritis were scored by the Hospital for Special Surgery score and the Knee Society score. Two blinded observers rated the patients independently in order to determine the concordance of the scores, the correlation between the overall scores and their subscales such as "pain",...

PURPOSE OF THE STUDYIntra-articular fractures of the proximal tibia always present a complicated therapeutic problem. In this retrospective study, the results of both conservative and surgical treatment of these fractures are evaluated.MATERIALIn the period from 1997 to 1999, 114 patients with proximal tibial fractures were treated in the Traumatological Hospital in Brno-Traumacenter. Of these, 34 (30 %) were treated conservatively and 80 (70 %) by a surgical procedure. A total of 80 patients were included in follow-up (70 %). Of these, 61 had surgery, which involved arthroscopically-assisted intervention in 25 patients, arthrotomy in 27...

PURPOSE OF THE STUDYThe aim of the study was to compare the results of revision surgery in diaphyseal fractures of the ulna and radius treated by intra-medullary interlocking nailing or plate osteosynthesis.MATERIAL AND METHODSThe group included 36 patients who, in the period from 2000 to 2005, were treated with the use of the intra-medullary ForeSight nail system (Smith & Nephew, USA) or a small, 3.5 mm auto-compression plate (different manufacturers). Pseudoarthrosis developed in 25 patients and recurrent fractures were found in 11 patients (six of these suffered fractures along the original fracture line after minimal trauma and...

The diagnosis of femoroacetabular impingement (FAI) syndrome is relatively recent. Therefore, the concept of early hip arthritis development is an interesting issue. The assumed prevalence of this disease is rather high and any postponement of necessity to perform total hip arthroplasty (THA) for hip arthritis, as a FAI sequela, should bring benefits to medical care as well as economy. To make an exact diagnosis it is necessary, in addition to essential clinical examination and standard conventional radiography, to carry out MRI that, in indicated cases, is combined with arthrography. Surgical treatment includes extensive procedures, such as controlled...

The author reports two cases of pediatric patients with supracondylar humeral fractures complicated by concomitant vascular injury. One of the patients also presented with neurological symptoms from compression of the ulnar and median nerves. In the case of vascular injury only, it was necessary to resect a 1-cm segment of the brachial artery which was thrombosed due to intimal disruption. In the other case, surgery was not indicated immediately; however, liberation of the nervus ulnaris and nervus medianus was later required because of nerve compression by the scar and bone. The author considers the exact diagnosis, precise reduction and stable...
