PURPOSE OF THE STUDYThe aim of this study is to evaluate the first experience gained with the new type of anatomical symphyseal plates intended to stabilise ruptured symphysis and closely located fractures.MATERIAL AND METHODSA retrospective evaluation was performed in 5 patients who had undergone surgery in the period from July 2015 to the end of 2016 using a new anatomical symphysial plate for pelvic ring injury. In four cases a stabilisation of symphyseolysis was carried out and in one case osteosynthesis of bilateral rami fracture near the symphysis was performed. The posterior pelvic segment was fixed 3 times by iliosacral screws and...

Acta Chir Orthop Traumatol Cech. 2018; 85(1):17-21 | DOI: 10.55095/achot2018/002

PURPOSE OF THE STUDYBone marrow oedema (BMO) syndrome is a multifactorial condition. Various conservative treatment options include analgesic therapy, immobilisation of the affected joint and/or systemic intravenous iloprost therapy. Many studies have confirmed the positive effect of iloprost therapy, but only after short-term follow-up. The purpose of this study was to show that treatment with iloprost leads to positive long-term functional and radiological outcomes for BMO of the knee.MATERIAL AND METHODSFifteen patients with BMO of the knee joint, ARCO stage 1-2, were included in this study. Various questionnaires, the Lysholm Score,...

PURPOSE OF THE STUDYThe aim of this paper was to compare terminal extension in normal and anterior cruciate ligament (ACL) deficient knees, and therefore to determine the role of the ACL during this motion.MATERIAL AND METHODSTen knees with ACL tears (7 knees with recent ACL tears, 3 knees with long-standing tears) and 10 normal contralateral knees have been examined using MRI in passive hyperextension, 20° flexion and 20° flexion with a 9 kg posteriorly directed load on the femur. Movements of the femoral condyles on the tibia were calculated using previously described methods.RESULTS1. Under the load at 20° flexion, knees with...

Acta Chir Orthop Traumatol Cech. 2018; 85(1):29-33 | DOI: 10.55095/achot2018/004

PURPOSE OF THE STUDYTreatment of comminuted three- and four-part displaced proximal humerus fractures continues to be discussed in daily trauma practice. In fractures with metaphyseal comminuted fractures the anatomical reconstruction is often technically unfeasible. For cases of comminuted metaphyseal proximal humerus fractures we proposed the so called non-anatomical reconstruction with simple osteosynthesis. Even today, when nailing and plating are commonly used in osteosynthesis, the non-anatomical reconstruction plays an irreplaceable role. Its application is conditioned by at least partially preserved vascular blood supply of the head fragment....

Acta Chir Orthop Traumatol Cech. 2018; 85(1):34-39 | DOI: 10.55095/achot2018/005

PURPOSE OF THE STUDYThe study evaluated the method of arthroscopic stabilisation of dynamic scapholunate (SL) instability by scapholunate joint resection using bone graft to create synarthrosis between the scaphoid and lunate.MATERIAL AND METHODSTwenty-six patients with chronic dynamic SL instability of Geisler-Messina - EWAS grade IIIB, C and IV were treated by artificial synarthrosis of SL joint using an arthroscopic technique. The mean follow-up period of the patients was 32.2 months (28-39 months, when the range of motion and grip strength were assessed. The functional results were evaluated through the Mayo wrist score.RESULTSPost-operatively,...

PURPOSE OF STUDYBased on the selected criteria the study presents the evaluation of data of patients who underwent a surgery for proximal femoral fracture at the Department of Orthopaedic Trauma of the 1st Faculty of Medicine and Military University Hospital Prague in the period 2013-2016. The paper aimed to determine whether there are any differences in proximal femoral fractures depending on the selected criteria.MATERIAL AND METHODSThe study comprised 805 patients (562 women and 243 men) older than 21 years with proximal femoral fractures who were treated between January 1, 2013 and December 31, 2016. In the study, the patient...

PURPOSE OF THE STUDYAseptic loosening of endoprosthetic components is the most frequent reason for total hip arthroplasty revision. This paper aimed to verify the influence of the type of hip-component fixation of total hip replacement in correlation with the age of patients on aseptic loosening of components during mid-term survival.MATERIAL AND METHODSThe retrospective, monocentric study statistically evaluated the data of 67 cases of implanted total hip replacements in 66 patients with the loosening of at least one of the components requiring a revision of the total number of 1,488 primary total hip replacements implanted during the...

Acta Chir Orthop Traumatol Cech. 2018; 85(1):54-56 | DOI: 10.55095/achot2018/008

PURPOSE OF THE STUDYWith the concept of the lateral compression plate (LLCP) a technique has been available designed to combine the advantages of a fixed-angle fixation with a complete sinking of the implant into the proximal bone. The objective of the present study was to investigate the results of the LLCP compared with classical screw osteosynthesis (SO).MATERIAL AND METHODS31 patients with pes planovalgus who received calcaneal displacement osteotomy and osteosyntheses with screws (n = 17) or LLCP (n = 14) between 2010 and 2015 were investigated retrospectively. The ankle-hindfoot scale, Kaikkonen score, VAS, and the SF-36 were determined...

Limitovaný versus extenzivní laterální přístup při osteosyntéze zlomenin patní kosti - srovnání časových a dynamických parametrů krokového cykluPůvodní práce

Acta Chir Orthop Traumatol Cech. 2018; 85(1):57-61 | DOI: 10.55095/achot2018/009

PURPOSE OF THE STUDYCurrently, operative treatment is preferred in dislocated calcaneal fractures. Most frequently used is the extended lateral approach, the disadvantage of which is the risk of early complications. Therefore, less invasive operative techniques are sought that would reduce the risk of such complications. The presented study aimed to compare the two different surgical approaches with respect to the restoration of gait stereotype using the objective pedobarography. We assume that the limited operative approach in osteosynthesis of intra-articular calcaneal fractures results in comparable or earlier restoration of gait stereotype...

PURPOSE OF THE STUDYThe aim of this study was to summarise the current approaches to diagnostics and therapy of the medial meniscus tear, and to evaluate the short-term results of meniscus repair by outside-in and all-inside techniques.MATERIALS AND METHODSThe study included 24 patients who underwent medial meniscus repair for vertical longitudinal tear 10 mm or longer in length localised in the rear two-thirds of meniscus in the red-red or red-white zone at the author's workplace between June 2012 and December 2014. As regards the method, either outside-in or all-inside techniques were used, or the combination of both. The Steadman technique...

Septic arthritis of the elbow seems to be a contraindication for total elbow arthroplasty (TEA). We here describe a 65-year-old male, American Society of Anesthesiologists (ASA) class 3 - patient, with a severely destructed right elbow due to septic arthritis with Staphylococcus aureus. His treatment consisted of multiple irrigation and debridement procedures including resection of the distal humerus, soft tissue coverage by local rotational flap and the use of a gentamicin-vancomycin loaded PMMA spacer, i.v. and oral antibiotics. After eradication of infection, a constraint cemented TEA could then successfully be performed eight months after...
