PURPOSE OF THE STUDYThe orthopaedic community has unanimously adopted the view that ultra high molecular weight polyethylene (UHMWPE) wear particles are a very frequent cause of aseptic implant loosening. Some studies have tried to provide objective evidence for this. We have found descriptions of particle distribution or morphology, but no report that would objectively correlate the number of particles in zones surrounding an implant with the extent of damage to these zones. The aim of this study was to develop a method allowing us to evaluate a number of samples with polyethylene abrasive wear large enough to find association between the extent...

PURPOSE OF THE STUDYThe aim of the study was to evaluate the group of patients operated on for chordoma at our department. We present diagnostic and surgical management options relevant to the localisation of chordoma in the spine, and evaluate the results in terms of post-operative complications, tumour recurrence and patient survival.MATERIAL AND METHODSBetween 1989 and 2002 a total of 20 patients diagnosed with chordoma were treated. There were 15 men and five women with an average age of 53 years. The cervical spine was affected in 20 %, thoracic in 15%, lumbar in 25 % and sacral in 40 % of the patients. All of them suffered from back...

PURPOSE OF THE STUDYThe study presents the mid-term results in patients treated with circumferential, i.e., 360-degreee, fusion of the lumbar spine carried out by posterolateral instrumented spondylodesis and anterior intersomatic fusion using a tantalum implant for replacement of an intervertebral disc. The aim of the study was to verify tantalum implant quality, to evaluate segment fusion and to assess the outcome of this method by patients' subjective findings.MATERIAL AND METHODSThe prospective non-randomised study included the implantation of 47 tantalum cages in 40 patients by the technique of anterior lumbar interbody fusion (ALIF)...

PURPOSE OF THE STUDYBone overgrowth of the femur after fracture in childhood treated either conservatively or surgically is well documented. The aim of this study was to investigate the frequency of bone overgrowth in childhood fractures treated by elastic stable intramedullary nailing (ESIN), to evaluate it in relation to conservative treatment and to compare its presence in the youngest age categories.MATERIAL AND METHODSA total of 49 patients, age range 4 to 17 years, with femoral shaft fracture treated in our department by ESIN osteosynthesis between 2001 and 2005 were asked to appear for a follow-up not earlier than one year after...

PURPOSE OF THE STUDYTreatment of leg length inequality via lengthening of the shorter extremity is an infrequent orthopedic procedure due to the requirement of special distraction devices and possible serious complications. Essential qualitative changes in operative technique development are associated with the name of G. A. Ilizarov, who paved the way for the autoregenerate gradual distraction method in the 1950s.MATERIAL AND METHODSIn the years 1990 through 2007 a total of 67 patients underwent femur lengthening via gradual distraction using various types of external fixators at the Department of Pediatric Surgery, Orthopedics, and Traumatology,...

PURPOSE OF THE STUDYThe aim of the study was to make a basic analysis of the patients treated for pelvic fractures in the year 2007 at 14 institutions in the Czech Republic and Slovakia.MATERIAL AND METHODSThe group investigated consisted of 474 patients treated in 2007. In a one-year prospective descriptive study, patients' data were recorded according to a unified protocol. They included age, gender, mechanism of injury, fracture type, associated injuries, method of treatment, post-operative complications, length of hospital stay, injury/surgery-to-standing time and number of deaths during primary hospitalization. The χ2 test of independence...

PURPOSE OF THE STUDYIn a retrospective study we evaluated the results of plate osteosynthesis for treatment of periprosthetic femoral fractures classified as Vancouver types B1 and B2.MATERIAL AND METHODSThe group comprised 19 patients with post-operative periprosthetic fractures treated by open reduction and internal fixation with plate osteosynthesis at our department between the beginning of 2004 and June 2007. Perioperative fractures were not included. The average age of the patients was 72.0 (range, 53 to 88) years. A locking compression plate (PCL) was used in 16 patients. The average follow-up was 21 months, with 6 months at least....

Acta Chir Orthop Traumatol Cech. 2009; 76(5):417-423 | DOI: 10.55095/achot2009/079

PURPOSE OF THE STUDYEach dynamic stabilisation should preserve motion at the operated segment as well as reduce a load on the disc and intervertebral joints. One of the methods to achieve this is the implantation of interspinous spacers between lumbar spinous processes. In this study, the patients treated with the DIAM interspinous spacer (Medtronic, USA) were prospectively followed up with the aim to evaluate clinical outcomes and post-operative complications.MATERIALPatients with a degenerative disease of the lumbosacral spine were indicated for the operation. They suffered from axial pain with signs of nerve root involvement due to disc...

Tibial plateau fractures can range from a simple lateral split pattern to very complex bicondylar injuries that can be a source of great disability. these fractures can provide a challenge for both junior as well as for senior surgeons alike. careful evaluation of the mechanism of injury and fracture pattern plays a crucial part in the operative planning. Assessment of the soft tissues shoul be performed carefully and adequate imaging is mandatory to allow a more detailed evaluation of the fracture architecture and pre-operativeplanning. Timely accurate reduction and fixation is the goal. Over the last decade the surgical approach chosen has been largely...

Spinal osteochondromas as solitary lesions are rare tumours of a maturing adolescent skeleton. The authors treated a 75-year-old man for low back pain and neurogenic claudication. Symptoms were attributed to a tumorous expansion originating from the spinous process and right lamina of L3 and expanding into the spinal canal and adjacent facet joints. The patient underwent marginal resection of the tumour together with transpedicular stabilization of the segment, and histological examination confirmed the diagnosis of osteochondroma. The patient remains without any complaint and there are no signs of local recurrence of the tumour 4 years after the surgery....

An unusual case of implant failure after revision total knee arthroplasty (TKA) is presented and the case report is completed with the results of an ensuing biomechanical study made to explain possible causes leading to a fracture of the tibial stem. For this we used electron microscopic and biomechanical analyses. There was no material defect found at the site of fracture. By computerized modelling of the clinical situation, however, we found out that asymmetrical progressive osteolysis of the proximal tibia could have caused significant changes in mechanical straining associated with qualitative alterations in the process of degradation and erosion...
